Arbor Place is dedicated to provide cost effective, quality assisted living care, while respecting the personal integrity of our residents. The Arbor Place team will nurture a foundation of independence, individuality and dignity with a choice of lifestyle. Family and friends, as appropriate, in care planning and implementation, will be facilitated while balancing a healthy, active and enjoyable residency in this safe, warm and home-like environment.
Arbor Place Resident Rights

♦ To be treated with courtesy and respect.

♦ To have privacy when receiving services or sharing information.

To make individual decisions regarding activities, use of space and time spent (unless activated
     Power of Attorney).

♦ To manage personal finances, unless other arrangements are made.

♦ To elect/decline any services included in the Service Agreement (as long as the refusal of services does
     not endanger others).

♦ To choose a personal physician and other service provider(s), consistent with policies and procedures
     of the facility.

♦ To furnish a private room and keep personal possessions, as space permits, if it does not unreasonably
     interfere with or endanger other residents.

♦ To receive visitors and meet with others as desired, including forming resident, group or family councils.

♦ To receive and send sealed, unopened mail.

♦ To have a telephone available for use at any time.

♦ To voluntarily participate or not participate in any religious activity.

♦ To have medical, personal and financial records kept confidential.

♦ To have confidential discussions with facility staff regarding physical, mental or medical conditions,
     services, payment sources and other personal affairs.

♦ To receive services consistent with the signed Individual Service Plan and Risk Agreement.

♦ To manage medications, unless otherwise stated in the Individual Service Plan or Risk Agreements. If
     the staff is managing medications, to receive all medications in timely intervals and proper dosages or
     to refuse medications, unless under court order.

♦ To live in a safe and secure environment.

♦ To be free from physical, sexual or emotional abuse, neglect or financial exploitation or
     misappropriation of property by facility staff or other service providers under contract with this facility.

♦ To file a grievance with this facility with no fear of retaliation.
