Arbor Place offers four levels of personal care and services, as well as Respite Care, to meet the individual needs of the residents. The level of care that is established for each resident is determined by a comprehensive assessment conducted through our medical staff and the resident's personal physician.
Residents are responsible for their own telephone services and personal effects, if they wish to have such items.
Level 1: Monitored Care
The Monitored Care plan is for the resident who completes their activities of daily living autonomously. Assistance with personal care is not required on a daily basis, but a supportive environment with basic services is provided. Basic services include:
* Private room with private bath, housekeeping and laundry services
* 24 hour supportive environment * Health monitoring, medications monitored by RN's/LPN's * 3 home cooked meals each day with snacks * Leisure time, activities, community events * Nursing, Social Worker, Chaplain
Level 2: Supported Care
The Supported Care plan is for residents who require frequent assistance with most activities of daily living. Along with the basic services, the resident may require assistance with bathing, grooming or personal hygiene. The resident requires the above Level 1 services plus one or more of the following services:
* Incontinence care
* Special dietary modifications * Safety risk related to weakness falls or other noted changes of condition * Occasional redirections and orientation * Assistance with dressing * Standby assist and/or 1 person transfer * Oxygen therapy with monitoring of oxygen levels * Foley catheter care
Level 3: Additional Supported Care
The Additional Supported Care plan is for residents who require the care listed above in Levels 1 and 2, and may require one or more of the following services:
* Set-up and 1:1 encouragement with meals
* Challenging behavior management * Consistent monitoring due to frequent falls and safety concerns (chair/bed alarms) * Ongoing redirection, orientation and monitoring * Diabetic care and management (scheduled blood sugars and/or insulin) * 2 person transfer * Ostomy care
Level 4: Extended Care
The Extended Care plan is for residents that require many of the above services and one or more of the following:
* Extensive hospice care
* Bed bound status * Mechanical lift * Assistance with all meals (not able to consistently feed self) |
